Registration by fax is available to anyone who wishes
to pay fees by VISA or MasterCard. To register by fax,
complete all sections of the continuing education registration
form. Fax the completed form to (850) 595-8485. Include
either your VISA or MasterCard account number, the expiration
date, and your signature in the designated space on the
form.
